Through the site

  1. Go to the Connect SUS website;
  2. Select the “Login” option;
  3. Enter your CPF number and Gov.br access password;
  4. Click on the orange button, located in the right corner of the screen;
  5. Wait a few moments for the document to appear on your screen;
  6. Finally, click on the “Download” option.

Through the Connect SUS application

  1. First install the Connect SUS application;
  2. Go to the menu and click on “Start”;
  3. Go to “Login”;
  4. Enter your CPF and then the Gov.br access password;
  5. Accept the terms of use of the platform;
  6. Tap the orange button, located on the bottom right corner of the screen;
  7. Click “Download”.

It is necessary to consider that, when applying for the SUS Card via the internet, the individual will have to have the physical card.

Those who do not have a record in the SUS will have to go to the Department of Health sector to request it. In this case, have documents such as RG, CPF, proof of residence and birth certificate.
