The student … is at the syllabic level of writing and has difficulty performing activities and monitoring the content.
He doesn't have a good relationship with the group, because every day there is an incident involving the student, but despite that, he is well accepted in the few moments that he shows some tranquility. The relationship with the teacher is satisfactory despite the difficulty the student has in dialoguing. He usually does not accept and understand the requests of adults and has not yet developed skills for socializing in school environment, as it demonstrates inappropriate behavior, always agitated, jumping around the room or kicking a friend. When not, he is out of the classroom, as he leaves without permission. The family has been notified.
Don't miss:
The student..., 8 years old, attending the 2nd year of elementary school at this school, is a child with normal behavior for his age. Has a good relationship with the group and with the teacher.
As for their development in the acquisition of reading and writing, there were no advances in this 1st quarter and for this reason a clinical evaluation was requested by the school board. The student is at the pre-syllabic level of writing, does not recognize the letters and demonstrates difficulties in speaking, substituting and omitting phonemes: casa-tasa, wall-paede, frog-chapo, porta-pota). Its speech is apparently childish and sometimes unintelligible.
Interventions are being made through stimuli such as daily reading, games and diversified activities at the initial literacy, and mainly, talking to the student and listening carefully, always giving a speech model right.
I am available if you need other information.
The student …………………… is a child who has a learning disability. It is hard work and for this reason it has been surpassing itself. According to daily observations, childish attitudes can be seen and changes in what is proposed to him, he does not seem to understand the commands. Interventions are made with different activities and family support with the reinforcement book that has been used since last year. Also, with individual monitoring by the classroom teachers and support, when possible.
As a student with good behavior and well supported by the family, we expect great results for the next quarter.
The student ……………………………………..presents difficulties in carrying out school activities. Every activity he starts cannot finish showing discouragement. You forget what you are told too easily. Some measures were taken such as parallel reinforcement, use of the mobile alphabet, different games and activities of the group, but there was no result, even with an individualized work, it presents great difficulty in concentration.
Despite the difficulties presented, he is a very active student during the games, he has a good relationship with the group, but he hardly responds to requests.
I'm waiting for new results in this 2nd quarter, to decide with the pedagogical group and the family, new directions for Matheus' case.
The student …………………….. resists participating in classes and doing what is proposed. He has a strong temper and often refuses to enter school. When present in the classroom, in addition to not participating, he insists on interrupting classes with jokes, shouts, shoving and cursing. As for learning, the student demonstrates difficulty and has not yet acquired the concepts, he is in the hypothesis of pre-syllabic writing, not recognizing letters and numbers. Despite several attempts as parallel reinforcement, diversified and playful activities, there is no enthusiasm, showing inattention, disinterest and indisposition. In group activities, he always finds a way to get into conflict with his colleagues, not having a good relationship with the class. He never keeps time and there's always a problem at recess. He has no whim with school supplies, scribbles and tears up books and notebooks, so much so that he changes notebooks weekly, in addition to spoiling the activities and materials of his classmates. In attempts to dialogue the student is indifferent, does not accept interventions and requests from adults. They use untruths to justify their actions.
Based on the daily observations described, I request a referral for medical evaluation.

The student …… is a very helpful child, he is always willing to help his peers and the teacher. He is supportive, he is always offering and sharing his belongings. In the school routine, he demonstrates difficulty in carrying out the proposed pedagogical activities, even the simplest ones, as he is always involved in conflicts.
His behavior is very agitated and he apparently feels the need to be on the move all the time. When proper posture is required for explanation, he moves his hands, feet, and bites the edge of his desk.
In the classroom environment he is always very disorganized, everything is on the floor around him, his materials and everything he can find along the way. It is never in its place, it crawls from one wallet to another. He still cannot use the notebook without guidance and is at the syllabic writing level. It recognizes individual letters and lists number / quantity.
Various interventions were made, diversified activities, individual support, activities with games and mobile alphabet, but he cannot concentrate and complete an activity.
Usually does not accept the requests of adults and follows the rules, making it difficult for him to get along at school. Despite being a very dear child, most of the time he is not accepted in study groups, games and games due to this issue.
He has already been warned for several undisciplinary reasons, among them, using force to get what he wants, demonstrating aggressiveness with his colleagues at these times. Its progress in the literacy process is also worrisome, as it does not carry out the proposed pedagogical activities and does not pay attention to the classes.
For the reasons mentioned and at the request of the student's mother, I request referral for evaluation in the health area.
The student …. presents difficulties in carrying out school activities. Every activity that starts cannot end up showing discouragement. It has not yet memorized the writing of its own name and does not recognize the letters and numbers. You forget what you are told very easily, in a matter of seconds. Some measures were taken as parallel reinforcement with the use of the mobile alphabet, games and activities differentiated from the group, but there was no result, even with an individualized work presents great difficulty in concentration. During activities on the blackboard or in the textbook, I notice that the student forces his eyes to see and write. Despite the difficulties presented, he is a very active student during the games, has a good relationship with the group and is initially interested in the proposed activities.
Based on the daily observations described, I request a referral for medical evaluation.
The student ………………… was transferred this 2nd quarter (07/04/2013) from a school in the mountains, she is a very agitated child, she has a good relationship with the boys (but she doesn't relate to the girls). When he arrived, I checked in his old notebook that the activities being proposed were inadequate and we started a new job. I deduced that his difficulty would be due to repetitive exercises with lowercase and cursive letters. With this new experience, we got a good step forward, but due to his restless behavior, doing everything by halves (he usually doesn't complete the words and sentences, even when copying), tearing the activities off the sheet and keeping your back all the time, looking for someone to talk to (in parallel) is showing great difficulty in advancing the writing hypothesis and reading. Another compromising factor is the absence of parental monitoring in their daughter's school life. She doesn't do the homework, I send notes that come back unsigned. When I call the older sister for treatment (she is in the 2nd year at this school) I realize that it is useless, as she also has difficulties and lack of follow-up.
I'm waiting for the parents' meeting to find out about the possibilities of ADHD and possible referral to the health area.
The student ………………… is an apparently calm child who tends to play the victim in various situations, even when he verbally attacks a classmate. It was relocated recently (08/05/2013) for the same reason. She behaves inappropriately, always crying, complaining of headaches and dizziness.
According to the mother, the student suffers from a disorder for being an only child. The student has difficulty, is taking the 1st year for the second time, is at the syllabic level and recognizes some syllables. She is dependent on the teacher in the simplest activities due to her difficulty in listening.
Due to the difficulties presented and the alleged health problems mentioned, I request a medical referral.
The student ……………… is a sweet and affectionate boy, he is at the syllabic level of writing. He exhibits childish behavior, speech and attitudes. She has a lot of difficulty concentrating because she is always playing with the materials, singing, talking to herself, she seems to be in another environment.
This quarter he made little progress, as he is always involved in his thoughts, completely disconnected from classes. He already writes his name, recognizes letters and numbers, knows how to use the notebook, but never completes an activity. He is a very sweet and quiet student. I believe that if more participatory, it could go much further.
Parents are aware of their child's behavior and school life and believe that there is still a long time for the student to become literate.
