She gets along very well with all her colleagues. He is a very calm, independent and creative child, being very organized with his individual and collective material.
She respects the rules and agreements (school and classroom). She has great behavior. Participate in all proposed activities with pleasure. At times she is shy. She is assiduous and punctual.

During this period, Analice had a good performance, getting involved with interest in the proposed activities.
She continues to establish a very friendly relationship with her peers and teachers. She is very attentive in class and tries to help her classmates whenever possible.
She collaborates in strengthening friendship and respecting the rules of coexistence at the School. In oral language, she manages to express herself with clarity and a logical sequence of ideas. In written language she can read and write simple words and sentences. In the area of logical thinking he does not present difficulties, skillfully performing addition and subtraction.
Most of the time, whether in carrying out activities, presentations and others, Analice shows up enthusiastic encouraging colleagues to actively participate in the activities proposed by me or their pairs.
Regarding homework, he continues to demonstrate a lot of responsibility and whimsy. It is necessary for the family to continue encouraging her, encouraging her to carry out the activities and guiding her in what is necessary. Likewise, it is necessary to value her achievements so that she can overcome her difficulties.
Don't miss:
This student has great difficulty in communication, does not socialize with colleagues, initially in February (beginning of classes) he was received at the class at very low level of performance – pre-syllabic with a lot of learning difficulty, did not identify letters or match sound/word.
During classes, the activities proposed for the class were dispensed by the student, he did not show interest or encouragement in solving them, the student also did not bring home activities resolved.
The teachers tried together with playful classes, home visits, meetings, individual conversations, tutoring, change the student's picture/diagnosis a little so that he/she feels inserted in the class.
Little by little there was a significant change in socialization, he started to participate more in the actions, he talked during reinforcement individual and could for a few moments read stabilized words of at most three simple syllables, in addition to learning the name complete.
He remains at the end of the school year, recognizing all the letters, formulating some words with the help of the teacher, speaking and writes the numbers and demonstrates possibilities for improvement, but not enough to act with writing or reading autonomy yet.
We believe that with a more constant and differentiated professional and family follow-up, the student can improve and advance in learning throughout the literacy cycle.

Usually student assessments are carried out bimonthly and recorded so that it is possible to check the progress or not of the student in different educational aspects, evaluating their participation in activities and also marking their attendance in classes in the report school. Student assessment can be carried out with or without final exams. But also in LDBE, there is talk of continuous assessment, that is, at every moment between student and teacher, he/she must be assessed.
Thus, the individual student report must reflect and contemplate the entire period of school activities, not just the time of exams.
LDBE - Law No. 9,394 of December 20, 1996
Establishes the guidelines and bases for national education.

Of course, this is just a template for a school report and you can post and make your own observations about the student in question. Ideally, you should print and make copies of the individual student report. Use the same sheet to write your additional observations about each and use the same sheet or sheets throughout the year to make a comparison.
You can create a header or cover for your school report with your school's name, your name, and a space to put each student's name and class.
Leave a space to paste a photo of the student too, this helps a lot for teachers who have many classes and can easily forget a name but not a face.
After all school reports are completed, you need to separate them, with the most serious cases that need attention and also the students who stand out, to also pay attention to them and continue with enough stimuli so that they don't lose their taste for to study.
Individual student reports can be passed on to teachers in subsequent years and also filed in the school archive for possible consultation.

You can use several methods of student assessment, being a traditional assessment (which you probably passed), or an innovative assessment, seeking to understand the student as a unique individual with their connections and interactions.
In Early Childhood Education, assessment plays the important role of providing elements for teachers to better know the children they work with, their personal and group characteristics, their emotions, reactions, desires, interests and ways in which they are appropriating the culture in which they are inserted, transforming it. One of the frequent questions that arise when writing the individual student report is in relation to the disabilities, which can be cognitive (learning) or physical (a limiting factor that prevents any activity). In this case, it is important that the student's school evaluation is done in a comparative and clear way, but in a way that also registers the progress and achievements obtained in the period evaluated.
In many cases, students with intellectual disabilities, in the individual report, the vision of all professionals participating in the educational process, discovering and creating new possibilities for learning.
Other cases of school evaluation reports where we need some special attention are the cases of undisciplined or deviant students.
In this case, in addition to the report, it is important to note whether these occurrences do not originate from some other social or family trauma. Each case must be treated individually and in a way that does not expose the student's identity to the entire school community. The monitoring of parents and guardians should be intensified and, if necessary, refer to the professional more suitable for guidance and/or treatment, in cases such as oppositional disorders, hyperactivity or violence extreme.
Well, that's it for now. And I hope you have taken advantage of this tip to facilitate your classes and your career as an Educator.
