We select individual report templates for special students. Teachers can follow the examples below when preparing each of their reports according to each student. Enjoy!
Model 1
The student …………………… is a child who has a learning disability. It is hard work and for this reason it has been surpassing itself. According to daily observations, childish attitudes can be seen and changes in what is proposed to him, he does not seem to understand the commands. Interventions are made with different activities and family support with the reinforcement book that has been used since last year. Also, with individual monitoring by the classroom teachers and support, when possible.
As a student with good behavior and well supported by the family, we expect great results for the next quarter.
Model 2
The student ……………………………………..presents difficulties in carrying out school activities. Every activity that starts cannot finish showing discouragement. You forget what you are told too easily. Some measures were taken such as parallel reinforcement, use of the mobile alphabet, different games and activities of the group, but there was no result, even with an individualized work, it presents great difficulty in concentration.
Model 3
The student …………………….. he is reluctant to participate in classes and do what is proposed to him. He has a strong temper and often refuses to enter school. When present in the classroom, in addition to not participating, he insists on interrupting classes with jokes, shouts, shoving and cursing. As for learning, the student demonstrates difficulty and has not yet acquired the concepts, he is in the hypothesis of pre-syllabic writing, not recognizing letters and numbers. Despite several attempts as parallel reinforcement, diversified and playful activities, there is no enthusiasm, showing inattention, disinterest and indisposition. In group activities, he always finds a way to come into conflict with his colleagues, not having a good relationship with the class. He never keeps time and there's always a problem at recess. He has no whim with school supplies, scribbles and tears up books and notebooks, so much so that he changes notebooks weekly, in addition to spoiling the activities and materials of his colleagues. In attempts to dialogue the student is indifferent, does not accept interventions and requests from adults. They use untruths to justify their actions.
Based on the daily observations described, I request a referral for medical evaluation.
